【摘要】Objective To discuss and investigate the pathogenesis,pathogenic bacteria,clinical manifestation characteristics,treatment effect and prognosis of delayed glaucoma bleb endophthalmitis.Methods Clinical data of 6 patients for delayed glaucoma bleb endophthalmitis were reviewed retrospectively.Results Six patients (6 eyes) suffered from delayed glaucoma bleb endophthalmitis after glaucoma filtering surgery postoperative 10 months to 2 years.All patients for delayed glaucoma bleb endophthalmitis were thin-wall filtering bleb.Among them,4 patients without empyema were performed intravitreal injection medicine,2 patients with vitreous body empyema were performed self conjunctive flap transfer filtering bleb prosthesis,vitrectomy and silicone oil filling.One patient,after intravitreal injection,whose inflammation and symptoms became worse,were implemented vitrectomy combined with silicone oil filling operation.Delayed-occurred endophthalmitis with glaucoma blebs of all 6 patients showed efficient after treatment.Conclusions Delayed glaucoma bleb endophthalmitis are bound up with thin-wall filtering bleb,intravitreal injection medicine and vitrectomy combined with silicone oil filling surgical treatment are highly effective.%目的 探讨青光眼滤过泡性迟发性眼内炎的病因、病原菌、临床表现、治疗及预后.方法 对6例青光眼滤过泡性迟发性眼内炎患者临床资料进行回顾性分析.结果 6例(6只眼)青光眼滤过泡性迟发性眼内炎发生在青光眼滤过术后10个月至2年,均为薄壁滤过泡患者,4例玻璃体未积脓患者行玻璃体内注药术,2例玻璃体积脓者行自体结膜瓣移植滤过泡修复术+玻璃体切割术+硅油填充术,1例玻璃体注药后炎症加重行玻璃体切割联合硅油填充术,6例眼内炎症均控制.结论 青光眼滤过泡性迟发性眼内炎的发生与薄壁滤过泡密切相关,玻璃体内注药及玻璃体切割联合硅油填充手术治疗有效.
