消毒餐具加工合同书 甲方： 乙方： 为改善市民公共饮食卫生条件和配合市食饮具卫生达标工作，双方本着平等协商，达成以下合同条款： 一、甲方为乙方提供餐具消毒服务。 消毒餐具质量标准符合国家标准GB14934-94《食饮具消毒卫生标准》并经当地卫生部监督机构检验合格后套装组合消毒餐具。 二、乙方为甲方提供：6寸平盘、4寸碗、江中杯、3号汤勺、9寸筷子、周转箱。 三、甲方根据乙方给予的库存餐具为乙方做好备货工作。 四、工作餐具在使用、配送、回收、清洗过程中会产生损耗，乙方需向甲方每个月提供一件汤勺和筷子1件，盘、碗、杯、周转箱根据实际损耗配补。 五、消毒餐具按照每套0.5元进行结算，每天支付，不得赊欠。 六、乙方须向甲方交纳2000元押金。乙方保证将收到的所有餐饮具交给甲方消毒清洗，乙方不得擅自操作。如发现违规，扣除所交押金。 七、本合同自生效之日起双方必须认真执行和遵守，在合同期内如发生合同外事宜，双方协商解决。如有一方违约，对方有权向法院进行起诉。 八、本合同有效期自 年 月 日至 年 月 日 止。合同一式两份，双方各执一份，签字生效后受法律保护。 甲方签字： 乙方签字： 年 月 日 奖品名称：500万U豆体验卡
