四色流式细胞术分析骨髓增生异常综合征原始细胞免疫表型

【摘要】  为了研究骨髓增生异常综合征(MDS)患者骨髓原始细胞免疫表型特征,应用四色流式细胞术对常规设门及二次设门策略选取的29MDS患者的骨髓细胞悬液中,CD34+细胞进行免疫表型分析。结果表明: ①随着MDS的进展(RA/RASRAEBRAEB-T),根据CD45/SSC图中选取的原始细胞群中,CD34+细胞比例逐渐增高,分别为8.0%46.4%76.8%,各亚型之间差别有显著意义(P<0.05)。CD45 vs SSC散点图上,设门所取的原始细胞抗原表达随着RA/RASRAEBRAEBT的进展,HLA-DRCD13CD33CD117表达逐渐增高,CD15表达逐渐减低,差别均有显著意义(P<0.05)。CD45 vs CD34散点图上二次设门取CD34+原始细胞行抗原表达的分析,同样异常表达HLA-DR及髓系抗原CD13CD33CD117,比例高于常规分析法(P<0.05),CD15比例减低,在RA/RASCD13CD33CD117HLA-DR的表达较常规分析的原始细胞比例明显增高(P<0.05),相关抗原的表达率在各亚型间差别无显著意义(P>0.05)。结论:常规设门法能反映MDS疾病的进展,而二次设门策略则更能反映MDS原始细胞的生物学特征,CD34的异常表达代表原始细胞的不成熟性和异质性,在临床高度怀疑但形态学和细胞遗传学不能诊断时,原始细胞免疫分型对MDS的诊断更具意义。

  Four-color Flow Cytometric Immunophenotypic Features of Blasts in Myelodysplastic  Syndromes

  AbstractThis study was aimed at exploring    the immunophenotypic features of blasts in patients with myelodysplastic syndromes (MDS). Four-color flow cytometry using conventional and secondary gating strategies was used to immunophenotype blasts and  the CD34 positive cells in bone marrow nucleated cells  of 29 patients with MDS. The results showed: (1) with progression of MDS from RA/RASRAEB to RAEB-Tthe proportion of CD34+ cells were gradually increased from 8.0%46.4% to 76.8% (P<0.05)(2) using CD45 vs SSC gating strategywith the transformation of RA/RASRAEB to RAEB-Tthe expression of HLA-DRCD13CD33CD117 were also gradually increased (P<0.05)and the expression of CD15 was gradually decreased (P<0.05)(3) using CD45 vs CD34 gating strategythe expression  of  HLA-DRCD13CD33CD117 on blasts were higher by secondary gating method than those by conventional gating (P<0.05). Howeverthere were no significant difference (P>0.05) in the expression of above-mentioned antigens on CD34+ cells among  different MDS subtypes. It is concluded that conventional gating method can  reflect MDS progression from RA/RASRAEB to RAEB-Tand secondary gating strategy may  accurately reflect the biological features of blasts in MDS. Abnormal expression of CD34 is related to the immaturity level and heterogeneity of blast cellswhich is beneficial to the diagnosis of clinically suspected MDS incapable of diagnosing with morphology and cytogenetics.

  骨髓增生异常综合征(MDS)是一组起源于造血干/祖细胞的恶性克隆性疾病,以外周血一系或多系细胞减少,骨髓一系或多系病态造血和高风险进展成急性髓细胞白血病为特征。我们应用一组单克隆抗体、四色六参数流式细胞术和常规和/或二次设门策略提取原始细胞、CD34阳性(CD34+)细胞,对其进行了免疫表型分析,以期为临床提供合理的诊断和治疗依据。

  材料和方法

  病例选择

  29MDS患者,男17例,女12例,年龄20-71岁,其中RA/RAS 17例、RAEB 6例和RAEB-T 6例。MDS的诊断参照《血液病诊断与疗效标准》(张之南,第2版)。

  材料

  单克隆抗体包括HLA-DR-FITCCD15-FITCCD2-FITCCD10-FITCCD20-FITCCD33-PECD13-PECD7-PECD19-PECD117-PECD34-ECDCD45-PY5等及其相应的标记荧光素的同型对照,所有单克隆抗体均为法国Immunotech公司产品。流式细胞仪为Beckman-Coulter公司的Epics XL 型,采用Expo32-ADC分析软件。

  骨髓原始细胞的表达

  对29MDS患者同时进行了形态学及免疫分型检测,其中26(89.7%)形态学诊断为MDS,此26例骨髓原始细胞在形态学上和流式细胞术上所占百分比见表1,两者无统计学差异(P>0.05)。余3例(10.3%)形态学无法诊断的患者免疫分型均提示为RA/RASTable 1. Correlation between analysis of flow cytometry and morphology in 26 cases(略)

  流式细胞术检测

  新鲜抽取的肝素抗凝骨髓(6小时内),调整细胞数至(0.5-1×106/ml,分6管标记抗体后,行流式细胞术分析测定。6管抗体组合分别为CD15/CD33/CD34/CD45HLA-DR/CD13/CD34/CD45CD2/CD7/CD34/CD45CD10/CD19/CD34/CD45CD20/CD117/CD34/CD45及阴性对照管IgG1-FITC/IgG1-PE/IgG1-ECD/CD45-PC5

  常规的分析方法

  在CD45 vs SSC散点图上,至少分析10 000个细胞,取常规原始细胞的位置设A门后对A门细胞行不同抗原表达的分析(图1)。

  二次设门分析方法

  T门为在CD34直方图上对A门原始细胞上行二次设门取得的CD34+原始细胞群,对T门细胞行不同抗原表达的分析(图2)。

  结果判定

 抗原表达结果为各标本管抗原表达率减去阴性对照管自发荧光表达率。CD34>5%,髓系、淋系抗原>20%为阳性。

  统计学处理t检验。

  结 

  免疫表型表达

  29MDS各种抗原表达结果见表2和表3。表2显示常规方法直接对原始细胞设门后的表达结果,表3显示二次设门后各种抗原在CD34+原始细胞上的表达结果。

  随着RA/RASRAEBRAEB-T的进展,CD34+细胞比例逐渐增高,分别为8.0%46.4%76.8%,各亚型之间差别有显著意义(P<0.05

  两种分析方法抗原表达

  检测结果显示,两种分析方法均异常表达HLA-DR及髓系抗原,不表达淋系相关抗原。常规获取的原始细胞随着RA/RASRAEBRAEB-T的进展,HLA-DRCD13CD33CD117表达逐渐增高,CD15表达逐渐减低,差别均有显著意义(P<0.05)。二次设门提取CD34+原始细胞同样异常表达HLA-DR及髓系抗原CD13CD33CD117,比例较常规分析结果增高,尤其在RA/RAS中,CD13CD33CD117HLA-DR的表达率分别为87%65.6%56.6%44.2%(表3),显著高于常规分析的原始细胞比例的51.6%37.6%17.1%23.3%(表2)(P<0.05),但上述抗原的表达率在MDS各亚型间差别无显著意义(P>0.05)。Table 2.
