寰枢关节半脱位的CT影像分析

        [摘要] 目的 分析寰枢关节半脱位的CT影像学特点。方法 回顾性分析该院自2016年1月—2018年1月期间收治的临床及影像资料完整的16例寰枢关节半脱位的CT影像学表现。结果 该组病例中寰枢关节半脱位主要有4种表现形式： ①侧方脱位；②旋转脱位；③前后脱位；④复合脱位。CT平扫可以清晰的观察到软组织及骨质结构，且各间隙的测量数无放大影响，寰齿前间隙增宽、寰椎向前滑脱、寰椎两侧块与齿状突间隙不等宽、寰齿后间隙缩小以及寰枢椎棘突间距增宽等征象是寰枢关节半脱位的主要影像表现。结论 熟悉寰枢关节的解剖结构，在CT扫描时采用标准体位，将寰枢关节半脱位的CT影像学资料与患者临床表现密切结合是提高该病早诊断、早治疗的关键。

        [Abstract] Objective This paper tries to analyze the CT imaging features of atlantoaxial subluxation. Methods A retrospective analysis of CT imaging findings of 16 cases of atlantoaxial joint subluxation treated in this hospital from January 2016 to January 2018 was completed. Results In this group of patients， atlantoaxial joint subluxation mainly has four manifestations：①lateral dislocation；②rotational dislocation；③anterior and posterior dislocation； ④composite dislocation. CT plain scan can clearly observe the soft tissue and bone structure， and the measurement of each gap has no amplification effect. The gap before caries widens， the anterior vertebrae slips off， and the gap between the two sides of the atlas and the odontoid gap is not equal. The main imaging manifestations of atlantoaxial joint subluxation include signs of narrowing of posterior caries， and widening of atlanto-axial spinous processes. Conclusion Familiar with the anatomic structure of atlantoaxial joints， the use of standard posture during CT scan， the CT imaging data of atlantoaxial joint subluxation and the patient's clinical performance are closely linked to improve the disease early diagnosis and early treatment.
